Electricity shut-off notice: help Kevin pay $900

The electricity account is 4 months behind. I received a disconnection notice that says service will be cut off on December unless I pay $900. I have never been late before August's medical emergency.
I called the provider and asked about a payment plan, but they need a down payment of $145 first, which I don't have.
I live with three children under ten, and going through the cold months without electricity isn't safe. We would also lose the ability to run the medical equipment at home.
I'm working on getting back on my feet, and will pay forward whatever help I can once I'm steady.
The money will go directly to the utility provider, and I'll post the paid statement here.
